Problems with Trial Account Request

Further to the video below, there are two problems with trial account requests submitted through the trial account request form:
  1. It is creating two trial account request for one form submission.
  2. Data entered into the Organization and Positions fields is not populations the relevant fields within the user account form.
Please resolve this as an Unplanned/Critical task.
