✔ Unable to create Draft and HTML is missing (UN/0073/04)

Dispute Document UN/0073/04 was published this morning. It had both PDF and HTML files.
Since publishing,- We are not able to create a draft version. I receive this message when I click "Create Draft"
- The PDF was showing an Error 404 message. This issue is resolved. Martin was able to add the PDF.
- The HTML does not appear on the admin site or live site. The data capture for this HTML was complete. (Please note that the data under the Analysis tab has no issues).
Please see the last three rows of the Document History tab. The Task Column is blank. Paul attempted to create a draft but was not able to: , are you able to retrieve the HTML from this morning? In other words, the HTML that had text highlighted for Analysis.

Also, note that both the HTML and the PDF files appear to have been physically removed from the app server, so I manually copied them back:

What Piyush did is correctly rename the HTML document I re-uploaded on the server yesterday. The name of the HTML file was not matching database records.
This still does not explain why the files (both PDF and HTML) went missing yesterday, and why we have the strange Task entries in Paul's screenshot above.

I tried to found but I got that as exceptional case the PDFs and HTMLs file were not saved in folder.
